RA 9262 §14 has the Punong Barangay issue a Barangay Protection Order — effective 15 days — on the date of filing, ex parte, and it's personally served by the Punong Barangay or a Barangay Kagawad immediately after issuance, so filing, issuance, and service can all happen the same visit.

Is it the barangay captain personally who issues it, or can any official do it?
§14 names the Punong Barangay as the issuing official. A Barangay Kagawad may serve it after issuance, but issuance itself is the Punong Barangay's.
If I want it to last longer than 15 days, do I also apply at the barangay?
No — going past 15 days means applying to the court for a TPO (§15, also same-day and ex parte, but 30 days) or later a PPO (§16, after notice and hearing, until revoked). The barangay's role under §14 is the BPO specifically.
