How is a TPO from the court different from a barangay protection order?

Short answer
Two things differ: who issues it, and how long it lasts. A BPO (RA 9262 §14) is issued by the Punong Barangay, on the date of filing, ex parte, and lasts 15 days. A TPO (§15) is issued by the court instead, also on the date of filing and also ex parte, but lasts 30 days.

Yes — both the BPO (15 days, §14) and the TPO (30 days, §15) are issued ex parte on the date of filing. What changes is the issuing authority (barangay vs. court) and the duration.
Do I have to get a BPO first before I can apply for a TPO?
The register doesn't describe a required sequence — §14 and §15 name two separate issuing authorities and don't state that one is a precondition for the other.
What comes after the TPO?
A PPO under §16, issued by the court after notice and hearing — unlike the BPO and TPO, both sides are heard — and it lasts until a court revokes it, not for a fixed number of days.
