What documents do I need to file a consumer complaint?

Last updated: 2026-07-11 ยท Educational content; not legal advice.

Short answer

The core set is the same across regulators and small claims: a valid government ID, the contract or agreement (loan disclosure, pawn ticket, terms of service), the receipts or transaction records, your written demand and proof it was sent, and any reply from the company. Add screenshots of harassing messages or advertised rates where relevant. For small claims you also file the verified Statement of Claim form and, where required, the barangay Certificate to File Action. Keep the originals; submit clear copies.

Frequently asked

What documents are always useful?

A government ID, the contract or agreement, the receipts or transaction records, your dated demand letter with proof of sending, and the company's reply if any. These form the backbone of a complaint with BSP, SEC, NPC, DTI, or a small claims court.

What if I was never given a copy of my loan terms?

Request one โ€” the Truth in Lending Act (RA 3765) requires the lender to give you a written disclosure statement of the finance charges. Its absence is itself documentable evidence of a disclosure violation you can cite in your complaint.

Do I need the originals or copies?

Keep your originals safe and submit clear photocopies or scans; bring the originals to a hearing if the court or agency asks to see them. Label each document and keep a personal set identical to what you filed.
